Sponsored Account online form submitted but the account has not yet been created

Sponsored Account requests cannot be processed until we receive the accompanying signed paperwork.

Please note that the original form can be found in the Sponsor's email account under the subject header "FORM SUBMISSION: Account Request". The email will be from noreply@cua.edu. The form needs to be printed and signed by the Sponsor (the person who completed the form) and the cognizant VP or Provost Office. Once signed, email the form to ts-security@cua.edu for processing.

If you no longer need the request, please respond to your original ticket letting us know and we will close the ticket. If you have sent the signed paperwork, please re-send to ts-security@cua.edu.
